Both GMC and EPIC will ask you for proof if the name on your degree and passport is different. This can be due to:
1. Your degree contains your father's name as middle name but your passport doesn't mention this. This is what you can do:
- Most of the counties, including India, mention father's name on the last page of your passport. Simply email them a copy of the same.
- Alternatively you can get a document signed by the authorised official of your medical school. The document MUST have the same line as mentioned:
This certifies that the names [name on document] and [name in ECFMG record] belong to one and the same person.
I had a similar problem and I sent both the above documents to GMC as well as EPIC.
2. Name change post marriage.
- Simply attach a copy of your marriage certificate.
3. Spelling error:
- Get the name changed on your degree if possible.
- In most of the cases,changing names on your degree becomes very difficult. So you can get the spelling of your name changed on your passport.
- Every country has different rules for getting the name changed.
- In India, you need to make an affidavit and publish your name change in 2 newspapers. With this proof, you will have to approach the passport office for a new passport to be made.
